Grant Application

To apply for funding, the Lute Foundation
requires the following documents and information:

  1. Official name of the Organization.
  2. Mailing address of the Organization.
  3. Phone number of the Organization.
  4. Listing of all Board Members or Officials and their respective addresses and phone numbers.
  5. Project overview with budget.
  6. Name and phone number of one contact person.
  7. Copy of issued 501(c)(3) tax identification certificate.
  8. Listing of all specific pledged contributions.
  9. Actual requested amount that your Organization is seeking from the Lute Foundation.
